Summary
Under the supervision of the Program Manager, the Housing Navigator for individuals enrolled in
Recovery Bridge Housing (RBH) is responsible for providing triage and linkage to support housing and
homeless services for adult singles.
Essential Functions

Job Description

Summary
Under the supervision of the Program Manager, the Housing Navigator for individuals enrolled in

Essential Functions
- Screen and triage individuals for available services through SPA 6 Clarity Database
- Conduct triage tools through Clarity
- Conduct intakes with eligible participants
- Develop Individual Housing Service Plans with each participant and conduct weekly updates
- Document all encounters with clients in HMIS within one business day
- Link and refer participants to available programs within HOPICS and SPA 6 CES as needed
- Identify community resources that will assist in removing barriers to meet immediate needs
- Maintain and monitor a master participant log to track
- Identify, report, and work to remove all barriers to housing stabilization
- Support clients in locating and applying for suitable housing based on their needs and the housing
- Provide weekly reports to Management on work completed
- Provide field-based case management services on a consistent basis
- Perform any other appropriate responsibilities as assigned by the Senior Manager and Program
- Maintain accurate files/records on activities, client services in compliance with HIPAA, CFR 42
- Attending meetings associated with other projects as assigned by the Program Manager.
- Monitor and track Housing Navigation referrals in HMIS
- Run HMIS report on housing navigators case notes every Friday to ensure all notes are up to date.
- S/he will assist in training and provide support to all onboarding staff, providers, and other
- Maintain appropriate boundaries with staff, clients and community partners.
- Adhere to SSG's Code of Ethics and HOPICS' Core Values.
- Represent the Agency in a professional manner at meetings and community events.
- Regular attendance required.

- High School diploma and 3 years' experience in the social service field OR associate degree in
