This position qualifies for a one-time $2,000 signing bonus, paid in two installments upon commencement of the position and after 6 months of employment (subject to change).Under the supervision of the Program Director and/or Program Coordinator, the Case Manager is responsible for but not limited to planning, facilitating, implementing, and coordinating mental health / case management services to adults with mental illness who are transitioning from locked placements (psychiatric and forensic) to community settings or living in permanent supportive housing.Essential FunctionsProvide individual rehabilitation and case management services by exploring community resources, providing linkage, and referrals to ensure continuous system of care.Coordinate and implement day rehabilitative program activities under the supervision of program coordinator.Coordinate with treatment team and facility staff members to provide intensive, structured services.Coordinate services with various public and private community health, mental health, and social service agencies in treatment of client.Report program activities and development, crises, and any other pertinent information to appropriate chain of commandTransport client(s) to connect with necessary community resourcesAssist clients, families; and significant others in understanding mental illness and gaining their acceptance of and participation in the treatment recommendations.Provide weekly contact with client to ensure continuous system of care.Participate and implement cooperative arrangements and/or team conferences with various inter-disciplinary team members in the development and implementation of client’s treatment plan and aftercare plan.Maintain and submit billing records in a timely manner.Maintain and update client’s files and documentation in accordance with DMH or DHS guidelines.Must drive to multiple job sites.Perform other tasks and responsibilities as assigned by the Program Director and/or Program Coordinator. * Minimum Qualifications - Knowledge, Skills and Abilities RequiredBachelor Degree in Social Work, Psychology or other related field from an accredited college or university or one year of related experience.One year of direct service experience in a hospital or mental health setting which has an interdisciplinary team approach.Legal status to work in the United States.Must have own reliable transportation, valid California Driver License, and proof of automobile insurance coverage.Fully COVID-19 Vaccinated with eligible booster(s)Knowledge: Basic Computer software programs (Experience on Microsoft Word and Excel, Internet); Knowledge and ability to relate to people with mental illness; Knowledge of DSM-IV and Mental IllnessSkills: Good writing and oral (communication) skills are necessary; Strong interpersonal and social skills; Time management skills necessaryAbilities: Ability to drive; Ability to develop and maintain effective working relations with program and with community representatives and contractors; Ability to work both independently and as a member of a team;Bilingual skills preferred but not requiredNon-Essential Qualifications (optional) - Knowledge, Skills and AbilitiesEnvironmental Conditions (Working Conditions)Job duties are to be performed both in office and field settings.Physical RequirementsThe employee is expected to drive, sit, stand, walk, carry a maximum of 20lbs, listen, and speak in order to complete job requirements.Mental RequirementsThe employee must be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.Company DescriptionSpecial Service for Groups, Inc. – better known as SSG – is a non-profit health and human service organization dedicated to building and sustaining community-based programs that address the needs of vulnerable communities.
