§ Summary

Under the direction of Intensive Services Manager, Bilingual Mental Health Rehabilitation Specialist will be responsible for Facilitating and will be responsible for providing Intensive Care Coordination (ICC) services including facilitation of assessments, care planning and coordination of services with other systems, guided by the needs of the child or youth. Bilingual MHRS will be responsible for coordinating Child Family Team (CFT)meeting to develop and guide planning and service delivery to meet the underlying needs of the child or youth and his or her family. Bilingual MHRS will implement strengths-based, individualized, family and youth-driven, and culturally relevant. Facilitator may provide mental health services such as, individual rehabilitation. Bilingual MHRS will act as a liaison with external agencies and convenes the Child and Family Team. The Facilitator takes the lead to ensure the team consists of family members, County representatives, the client, and others selected by the family. The Facilitator works to ensure all team members are active participants in the CFT Process. The Bilingual MHRS will work with family to generate Mission statement and list of underlying needs, while assisting child/family and team to identify and prioritize needs. Collaborates with team members to ensure adherence to program process and principles.

§ Essential Functions

§ Gathers and updates information regarding the family’s strengths, needs, history, and past trauma experience

§ Develops a CFT meeting agenda

§ Coordinated and scheduled the staff and family engagement meetings

§ Outreach and engaged formal and informal supports

§ Facilitates the CFT meetings with the child or youth and family if the child, youth and family agrees

§ Facilitates the debrief with the team members, including the Children’s Social worker

§ Collaborates in the development of trauma related underlying needs statements (for youth and parent or caregiver )

§ Bilingual MHRS will be in charge of managing the logistics of the CFT Meeting process

§ The facilitator will guide the CFT Meeting process with the goal of addressing the child’s and family’s underlying needs as well as their strengths

§ Attend all necessary meetings, trainings as required by Supervisor, Director, or program contract.

§ Act as liaison between family members/identified supports, service providers, Department of Children and Family Services (DCFS) personnel, client’s lawyers, and the community for the purposes of providing case management, advocacy, needed resource information and linkage, and highly coordinated services as directed by the Child and Family Team (CFT)

§ Assists in maintaining on-going relations with County agencies, health care facilities, network agencies, placement centers, schools and other resources necessary to affect the continuum of care for children in the community.

§ Adhere to Los Angeles County Department of Mental Health (LAC-DMH) and OTTP Mental Health Policy and Procedures for the billing and documentation practices and maintain minimum productivity expectations

§ Ensures the Integrated Core Practice Model are adhered to by all team members, including the identification of client and family strengths, the providing of services in a timely and appropriate manner, and being the primary contact point for children, families, service providers, and community resource personnel

§ Perform outcome measure application describing level of psychosocial impairment, physical health, self-care, potential support network, adequacy of living arrangements, financial status, employment status and potential training or educational needs.

§ Demonstrate an ability to work professionally and independently with limited supervision, as well as an ability to exercise professional discretion and independent judgment based on training, education, experience and personal creativity

§ Demonstrate professional and effective communication abilities, including the completion of written reports, letters and plans, and initiating and responding to verbal communications with clients, families, the CFT and inter and intra-agency personnel

§ Reports to Clinical Supervisor immediately if Facilitator suspects a child is in danger

§ Maintain a working knowledge of existing community programs, the services provided and eligibility requirements

§ Assists in “Whatever it takes” approach such as counseling, shadowing, modeling, skills training, and other interventions deemed necessary, to prepare youth for successful and lasting housing, vocational, educational, and treatment services

§ Perform any other appropriate duties assigned by the Program Manager

§ Attend all monthly staff meetings

§ Participate in the Full Partnership Services crisis phone line rotation

§ Work on other appropriate tasks as assigned by the Manager

§ Driving is a required aspect of the job (for example: must drive to multiple field sites).

§ Regular attendance required & Other duties as needed

§ Fully Covid-19 vaccinated (2 weeks from 2nd dose)

§ Secondary Functions

§ Overtime, holiday, or weekend work may be required

§ Minimum Qualifications - Knowledge, Skills and Abilities Required

§ Masters Degree in Psychology, Social Work, Counseling or related field plus two (2) years’of experience in a mental health setting OR

§ Bachelor’s degree in Psychology, Social Work, Counseling or related field plus (4) years’ of experience in a mental health setting

§ Two-Four years of clinical experience in a recognized social service agency, clinic, hospital, or public health agency involved in problems of mental or emotional adjustment and the use of psychiatric consultation, depending on degree level.

§ Experience working high-risk children, adolescents, adults within Mental Health, Educational, Child Welfare and/or Juvenile Probation and/or Juvenile System

§ Bilingual ( English /Spanish)

§ Demonstrated ability to work with at-risk population

§ Knowledge and ability to work with at–risk population

§ Excellent communication skills

§ Excellent documentation skills

§ TB test clearance

§ Ability to function as a member of an interdisciplinary team

§ Valid California driver’s license, DMV check, reliable vehicle and proof of current automobile insurance required.

§ Background check & Verification of employment eligibility

§ Ability to perform duties on some weekday evenings and weekend days when required

.

§ Environmental Conditions (Working Conditions)

The environment for this position is an office environment as well as working within at-risk areas including home and school based services. Driving is required when in the field, own transportation is required.

§ Physical Requirements
