Job Title: Vocational Transition Services Facilitator (.20 FTE)

Position Type: Part-Time (.20 FTE) – School Year 25-26
Job Summary:
The Vocational Transition Services Facilitator supports students with special needs in preparing for post-secondary life through interest inventories, transition assessments, and coordination of services in alignment with each student’s Individualized Education Program (IEP). This position is responsible for delivering tailored transition support, facilitating parent communication, and providing referrals to appropriate outside agencies.
Key Responsibilities:
- Career Exploration & Assessment:
- Administer annual career interest inventories to all AJHS and high school special education students.
- Conduct post-school transition assessments aligned with IEP goals.
- Provide post-secondary guidance tailored to each student’s strengths, preferences, and interests.
- Senior Student Support:
- Guide seniors through the college and career planning process, including:
- College exploration and application assistance
- FAFSA application support, if applicable.
- Regular follow-ups to ensure transition goals are on track
- Parent & Agency Coordination:
- Schedule and conduct appointments with parents/guardians to discuss transition planning and available resources.
- Offer referral information and establish communication with relevant community and vocational agencies to support transition planning.
- IEP Support: Collaborate with case managers to ensure vocational and post-secondary goals are appropriately addressed in student IEPs. Maintain accurate documen tation of all assessments and planning activities.
Qualifications:
- Valid teaching license in special education (preferred)
- Experience in post-secondary planning with students with disabilities
- Strong organizational and communication skills
- Familiarity with IEP documentation and transition planning requirements
- Ability to collaborate effectively with students, families, school staff, and outside agencies
