Clinical Social Work/Care Coordination
40 hour/week position divided between direct billable mental health and care coordination services.
3 Onsite Days As Follows:
Monday- Spaulding Salem
Tuesday- Spaulding Boston
Thursday- Spaulding Lexington
Work directly with pediatric patients (and their families) in the outpatient rehabilitation setting who are injured or disabled, ill, or newly diagnosed with a pediatric condition and who may experience life altering changes in income, health insurance coverage, physical access to their environment, specialized transportation, and caregiver support to maximize independent function in the community. Collaborate across the continuum of care and with the interdisciplinary rehabilitation teams, community providers, and community agencies to promote the highest level of independent function and an active, productive, and fulfilling life. Direct and assist patients and their families in the application process for federal, state, and private resources and provide guidance to specialized community resources.
Additionally, the ideal candidate will be experienced in assessing and providing therapy to patients with diverse clinical conditions including anxiety, depression, OCD, ADHD, Autism, Attachment, trauma (PTSD), developmental delay and often within the context of complex family systems and/or medical compromise.

Work independently, be self-directed and flexible, and contribute as a member of a team.
