Position Summary

Responsibilities include pre-screening, enrollment, and recertification for the AccessHealth program, conducting home visits, and accompanying clients to appointments as needed. Working under the direction of a Registered Nurse and/or Social Worker, CHWs collaborate with healthcare providers, community organizations, and support networks to ensure smooth care navigation and optimal health outcomes.

Minimum Requirements
Education
- Associate Degree or 5 years experience in community health or related field
- 1 year experience case management or care coordination/navigation or community health in a paid or volunteer capacity
- Must receive SC Community Health Worker certification within 1 year of hire
- Valid SC driver’s license with a good driving record
Preferred Education
- Bachelor’s degree in community health or related field
- 3-5 years case management, care navigation, or community health experience, or related field
- Bi-lingual (Spanish) speaking preferred
- SC Community Health worker certification
Direct Client Services
- Manage a caseload of clients, including enrollment, recertification, and ongoing engagement.
- Conduct assessments to identify health and social needs; support clients in developing individualized care plans.
- Provide outreach and coaching to help clients overcome barriers and adopt healthy behaviors.
- Meet clients in various settings, including home visits and appointments, as needed.
- Educate clients and caregivers on accessing healthcare appropriately and managing health needs.
- Promote self-management skills through individualized coaching and resource navigation.
- Advocate for clients by connecting them to community resources and support networks.
- Build and maintain positive relationships with clients, providers, community partners, and the AccessHealth team.
- Proactively collaborate with healthcare providers and community resources to support client goals.
- Identify opportunities for improvement and communicate them through coaching and feedback.
- Maintain accurate and timely documentation of all client interactions and care coordination activities.
- Achieve productivity and documentation standards set by the supervisor.
- Ensure compliance with organizational policies and confidentiality requirements.
