Responsibilities
- Assist clients with daily activities such as bathing, dressing, grooming, and mobility to support their independence.
- Prepare nutritious meals and help with light housekeeping tasks to maintain a clean, safe living environment.
- Provide companionship and emotional support to reduce feelings of loneliness and promote mental well-being.
- Support clients in engaging with community activities or hobbies that enhance their quality of life.
Experience
- Previous experience in home health care, personal caregiving, or related roles is preferred but not required.
- Strong interpersonal skills with a genuine desire to help others thrive in their daily lives.
- Ability to follow detailed care instructions accurately and maintain confidentiality at all times.
- Reliable, punctual, and Honest
- Basic knowledge of health and safety standards relevant to home caregiving roles.
- Valid License and reliable transportation.

Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.
Self-direction allows participants to make their own choices, selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
