Southwoods Health at Home is seeking a dedicated Physical Therapist to provide high-quality, patient-centered care directly in the home setting. You will lead the evaluation and treatment process, helping patients restore function, relieve pain, and prevent disability following injury or surgery.
Essential Duties:
- Clinical Evaluation: Review physician referrals and medical records to develop and revise comprehensive, individualized treatment plans.
- Direct Care: Administer prescribed physical therapy to improve motor development, functional capacity, and respiratory/circulatory function.
- Multidisciplinary Collaboration: Partner with physicians and other practitioners to integrate therapy into the patient’s total healthcare plan.
- Supervision: Provide professional oversight for Physical Therapist Assistants (PTAs), students, and other licensed/unlicensed personnel.
- Patient Education: Empower patients and their families with information on post-op recovery, physician protocols, and home exercise programs.
- Documentation: Maintain accurate, timely patient records in compliance with Medicare and federal/state regulatory standards.
- Education: Graduate of an accredited Physical Therapy program.
- Licensure: Active Physical Therapist (PT) license in the State of Ohio.
- Certification: Current BLS certification through the American Heart Association (AHA).
- Experience: Previous experience in Home Health therapy is highly preferred.
- Skills: Exceptional interpersonal skills and the ability to work independently within a multidisciplinary team.
