Overall Function:
Performs a variety of tasks and functions required by the agency for successful operations and expeditious delivery of quality services.
Essential Functions:
- Complete initial referrals for Caregiver Support and Dom Care programs.
- Receives, processes, and completes all documentation of Caregiver Support Program invoices, including but not limited to uploading file attachments and enters service deliveries.
- Prepares and submits all required program forms, documentation, and correspondence in an accurate and timely manner to ensure Caregiver Support Program compliance.
- Prepares, maintains, and tracks requirements for Dom Care Providers and Consumers including but not limited to Provider Certifications, Rental Agreements, Rent Rebates, insurance information, etc.
- Responsible for troubleshooting and correcting discrepancies on program reports.
- Assist Care Managers with completing required telephone contacts with consumers.
- Contributes to and maintains current records as well as documentation in Social Assistance Management System (SAMS) for all case activities.
- Assist Care Managers with other program related tasks as necessary.
- Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
Qualifications:
- Associate Degree in Human Services or Business Management from an accredited college and one year of Aging experience preferred; any relevan combination of skills, experiences and training may be considered at the discretion of the agency.
- Demonstrated knowledge of Department of Aging Program regulations and policies and other appropriate government regulations.
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
- Must successfully obtain Criminal History Clearance from PA State Police and Child Abuse Clearance from the PA Dept. of Human Services, and FBI Fingerprint Clearances.
- Demonstrated ability to organize, coordinate and manage multiple responsibilities.
- Demonstrated knowledge of financial entitlement programs and available social service programs
- The ability to establish and maintain effective working relationships with the public.
