Responsibilities:
- Accurately assemble, analyze, and organize medical records for both paper and electronic formats.
- Ensure that all patient information is appropriately documented, filed, and updated in compliance with established protocols and regulatory guidelines.
- Review, scan, and index incoming medical records and documentation into the electronic health record (EHR) system.
- Organize and maintain physical filing systems, ensuring easy retrieval and accessibility of patient records when required.
- Retrieve and release medical records to authorized personnel in accordance with HIPAA and other relevant privacy laws and regulations.
- Collaborate with medical staff, nurses, and administrative personnel to retrieve and update patient information as needed.
- Assist in the auditing and quality control of medical records to ensure accuracy and completeness.
- Process requests for release of information from patients, legal entities, and healthcare providers, adhering to appropriate consent and authorization procedures.
- Assist in the training of new staff members on medical record management procedures and practices.
- Provide excellent customer service to patients, families, and healthcare providers regarding medical record requests and inquiries.
- Participate in departmental meetings and contribute to process improvement initiatives aimed at enhancing record-keeping efficiency and accuracy.
- High school diploma or equivalent; post-secondary education or relevant certification is a plus.
- Prior experience in medical records management, preferably in a healthcare setting.
- Familiarity with electronic health record (EHR) systems and medical record software applications.
- Strong knowledge of HIPAA regulations and patient confidentiality protocols.
- Proficient computer skills, including data entry, scanning, and electronic document management.
- Excellent organizational skills with the ability to multitask and prioritize tasks effectively.
- Detail-oriented approach to work, ensuring accuracy and completeness in record-keeping.
