Job Duties
- Vital Record Processing and Issuance: Processes and issues all types of certificates (Birth, Death, Marriage, Divorce, and Stillbirth). This includes reviewing legal documents to determine issuance rights, and ensuring the timely, complete, and accurate registration of deaths.
- Documentation Review and Correction: Examines documents and applications for completeness, accuracy, and validity; utilizes the Utah Department of Health digital system for vital records (including EDEN); processes necessary corrections to records (including preparing affidavits); and works with State Vital Records to resolve problems.
- Financial and Reporting Management: Accounts for numbered certified paper, certified copy forms and fees collected; receives, receipts, and balances/reconciles payments and accounts. Prepares and submits daily, monthly, activity, and fiscal reports as required by the department or the Utah Department of Health.
- Confidentiality and File Maintenance: Requests and handles highly confidential information from other agencies, ensuring confidentiality protocols are met. This includes maintaining confidential files in a locked room, reviewing files for accuracy, and updating/purging files as necessary.
- Customer Service: Provides comprehensive customer service (in person and via phone), answering inquiries and responding to complaints (including emotional customers). Assists the public with filling out request forms (for all certificates and Voluntary Declaration of Paternity forms), explains affidavit procedures.
- Permits and Mortuary Relations: Prepares and notarizes documents, prepares disinterment and transit permits for bodies shipped out of state for mortuaries, and prepares monthly invoices to mortuaries.
Skills/Abilities
- Computer skills (google drive, Microsoft Suite, etc)
- Must be able to multitask
- Good customer service skills, ability to communicate effectively verbally and in writing
- Spanish speaking STRONGLY preferred
Minimum Qualifications
- Graduation from high school or GED
- Must possess a Utah valid driver’s license
Preferred Qualifications:
- Ability to serve as Notary Public
Work Environment
May be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normally assigned duties during the incident and/or emergency.
