Shift Details: Direct Support Professional (Caregiver)
- Monday: 8:30 AM–4:30 PM – Shared Support
- Tuesday: 8:30 AM–4:30 PM – Individual Support
- Wednesday: 8:30 AM–4:30 PM – Shared Support
- Thursday: 8:30 AM–4:30 PM – Individual Support
- Friday: 8:30 AM–4:30 PM – Shared Support
Total Schedule: Up to 40 hours per week
Core Responsibilities: Direct Support Professional (Caregiver)
- Personal Care: Assist consumers with daily routines and personal care needs as outlined in their individual service plans.
- Home Support: Assist with meal preparation, light housekeeping, and maintaining a safe and comfortable living environment.
- Daily Activities: Provide support with daily routines and activities while encouraging independence and personal choice.
- Community Support: Assist with community activities and appointments when required.
- Daily Documentation: Maintain accurate documentation of services provided and communicate any concerns or changes according to SLIC procedures.
Requirements: Direct Support Professional (Caregiver)
- Age: Must be 18 years of age or older.
- Documents: Valid Driver’s License or Valid ID and Social Security Card
- Background: Must pass all state-mandated background checks and screenings.
- Transport: Reliable transportation to consumer homes in Jennings may be required depending on specific position.
- Character: A patient, compassionate, and reliable work ethic.
