Position Title: Preservation Coordinator: – Full-Time- Hours: Monday through Friday 9 am to 4 pm
Annual Salary: $40,000 Reports to: Executive Director
Job Description:
Primary Functions include the following:
Assist families/individuals in the Southwest Community who are experiencing an emergency situation and to develop a long-term continuum of support services that will prevent the recurrence of the emergency situation.
- Assist clients in completing an assessment application
- Collect all necessary required information to determine eligibility
- Provide an immediate resolve to the present crisis
- Identify personal/family strengths and appropriate community resources
- Provide an action plan for achieving a higher level of self-sufficiency
- Provide case-management and follow-up for support services and advocacy to ensure that gains are maintained.
- Provide action steps for families/individuals to help identify action steps that they can take to help prevent further emergencies.
- Contact families, 30, 60, 90 days to determine progress they have made on their action steps
- Communicate and meet with community agencies and food cupboards on a regular basis
- Make proper referrals where needed to assist in preventing recurrence of basic needs emergencies, i.e. food, clothing
- Determine if longer-term support program services are needed and make proper referral and follow-up
EDUCATION OF CLIENTS:
- Set up 5 workshops for at least 550 individuals per year in the areas of credit counseling, foreclosure prevention, debt consolidation, mortgage/grant assistance and first-time home buyers' programs.
- Set up appointments for families/residents to have CASH assist with their yearly tax returns.
QUALIFICATIONS/REQUIREMENTS:
- Must have high school diploma or GED. Minimum 3 years Human Service experience
- Excellent organizational planning skills
- Excellent oral and written communication
- Excellent decision making
- Ability to work independently
- Computer skills and knowledge in computer software programs: Microsoft Word, Windows, Excel
- Excellent people skills (teamwork) high level of professionalism
- Ability to define problems, collect data, establish facts and draw valid conclusions
- Ability to maintain confidential information
- Perform duties in a professional manner
- Be a positive role model for all.
Position may require some overtime, weekend and night scheduling depending on the needs of the client.
Any additional duties as needed for the advancement of the program/agency outlined by the Executive Director.
