SUMMARY:
Under general supervision of the Manager of the Diabetes Learning Center (DLC); provides out patient diabetes education at Tift Regional Health Systems, satellite clinics and/or community groups within the TRHS service area; functions as a member of an interdisciplinary team to assess diabetes care skills and education needs of clients and/or care providers; provides self-management education to assist clients in maintaining health and to prevent or manage complications; coordinates the Diabetes Support Group program; and provides In-Patient diabetes education as needed. Teaching experience desirable.
RESPONSIBILITIES:
* Assesses patient and/or caregiver(s) diabetes self-management skills & knowledge using the appropriate encounter in Cerner EMR (electronic medical record); selects & completes the correct assessment 100% of the time.
- Provides education to patients and/or caregivers based on the individual needs assessment following the written DLC curriculum, and documents or assures appropriate documentation of education provided in EMR 100% of the time.
- Conducts educational follow-up with patients and/or caregivers based on DLC education curriculum; selects & completes, or assures completion, the correct follow-up encounter in EMR 100% of the time.
- Conducts diabetes self-management assessment and provides education appropriate to the patient's and/or caregiver(s) age, cognitive level and physical ability.
- Communicates accurately and effectively with physicians or other healthcare professional regarding patient progress with diabetes self-management skill development by phone or written communication. Assures DLC policy regarding written communication to referring physician is followed 100% of the time.
- Completes, or assures completion of, charges for patients within 24 hours of the chargeable service.
- Follows appropriate DLC computer down-time procedures for patient assessment, documentation of education, documentation of physician communication and charging for services when computer access is unavailable.
- Cross-trains to provide & properly document In-patient diabetes education and to serve as a resource to physicians or nursing staff to promote glycemic control. Knows and can provide guidance for the implementation of all In-patient glycemic control physician order sets and/or protocols used at TRHS.
- Keeps abreast of pertinent federal, and state regulations and laws and Tift Regional Health System, Inc. (“TRHS”) policies as they presently exist and as they change or are modified.
- Understands and adheres to: TRHS’ compliance standards as they appear in TRHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy; and HIPAA and TRHS policies regarding privacy and security of protected health information.
- Demonstrates the ability to perform tasks that meet the age-specific requirements of the persons, patients, vendors, and staff that the employee is charged to interact with as required by the position.
- Offers suggestions on ways to improve operations of department and reduce costs.
- Attends all mandatory education programs.
- Improves self-knowledge through voluntarily attending continuing education/certification classes.
- Maintains required competency levels as identified in written exams, skills checklists, skills labs, annual safety and health requirements as well as service excellence education hours requirements.
- Cross-trains in order to better assist co-workers and to provide maximum efficiency in the department.
- Volunteers/participates on hospital committees, functions, and department projects.
- Manages resources effectively.
- Reports equipment in need of repair in order to extend life of equipment and removes malfunctioning equipment out of service with timely reporting to the appropriate personnel.
- Makes good use of time so as to not create needless overtime.

* Associate's Degree in Nursing

* REGISTERED NURSE
OTHER INFORMATION:
Must maintain current licensure as a Registered Nurse in the State of Georgia. Certified Diabetes Educator (CDE) preferred. Minimum of two (2) years work experience required. Bilingual preferred.
