Position Summary:
The Centering Coordinator is responsible for the planning, coordination, and implementation of Centering Pregnancy and Centering Parenting group care models across multiple sites. This role ensures high-quality group facilitation support, participant engagement, and program fidelity while working closely with clinical teams, outreach staff, and community partners. The coordinator plays a key role in improving maternal and child health outcomes by supporting group-based care, increasing enrollment, and ensuring a positive and consistent participant experience.
Key Responsibilities:
- Program Coordination & Implementation
- Coordinate and manage Centering Pregnancy and Parenting groups across multiple sites
- Develop and maintain group schedules in collaboration with providers and site leadership
- Ensure rooms, materials, and supplies are prepared for each session
- Support facilitation of sessions alongside clinical staff when needed
- Maintain fidelity to the Centering model and guidelines Participant Engagement & Enrollment
- Recruit and enroll eligible participants into Centering programs
- Conduct intake, screening, and orientation for new participants
- Maintain ongoing communication with participants (calls, texts, reminders)
- Track attendance and follow up on missed sessions
- Create a welcoming, supportive, and engaging group environment Collaboration & Team Support
- Work closely with providers, nurses, social workers, and outreach teams
- Coordinate with front desk and site staff to support scheduling and patient flow
- Partner with outreach staff to promote programs at community events
- Support integration with Healthy Start services and referrals Data Tracking & Reporting
- Track enrollment, attendance, and participation metrics
- Maintain accurate documentation in systems (HER, Teams and Excel)
- Assist with reporting requirements and data quality initiatives
- Monitor progress toward program goals (e.g., group participation targets) Multi-Site Coordination
- Travel between multiple clinics
- Ensure consistency in program delivery across all sites
- Identify site-specific needs and implement improvements
- Support new site launches and program expansion Program Development & Quality Improvement
- Assist in planning new Centering groups and expansion strategies
- Gather participant feedback and implement improvements
- Support accreditation readiness (e.g., Centering Healthcare Institute)
- Participate in team meetings, training, and program evaluations
Minimum Qualifications:
Education:
- Health Educator, Nursing, Medical Assistant or related field (preferred.)
- Equivalent experience may be considered
Experience:
- 2 years of experience in maternal and child health, group facilitation, or care coordination
- Experience with Centering Pregnancy/Parenting or group-based care models preferred
- Experience working in community health, FQHC, or public health setting strongly preferred

Preferred Qualifications:
- Certified or trained in Centering Pregnancy/Parenting model
- Bilingual (Spanish or other languages relevant to patient population)
- Experience with maternal-child health programs or Healthy Start
