What are the responsibilities and job description for the Behavioral Health Care Coordinator position at Southside Community Health Services?
- Answer calls for behavioral health department in timely manner. Respond to voicemails within 24 hours.
- Schedule initial patients seeking behavioral health services by determining their reason for seeking services and ensure they are set up with appropriate provider based on need.
- Maintain waitlist if providers schedules are full.
- Assist with follow ups/rescheduling needs.
- Assist in developing resources, workflows, handouts, etc.
- Serve as a bridge between the patient and internal referrals.
- Enter and maintain accurate patient information in the EHR to ensure seamless documentation and billing, including all telephone calls made.
- Prepare for next day visits through pre-visit planning process, including ensuring patient’s insurance is active, completing sliding fee verification, and collecting nominal fees.
- Collect payment prior to telehealth visits and set up payment plans as necessary.
- Ensure patient exam rooms are fully restocked with BH business cards, DV resources, child development resources, post-partum education and self-harm resources.
- Create intake packets and ensure they are ready prior to visit and at front desk. This includes seeing if patients’ screenings need updating.
- If patient is eligible for insurance or needs help with insurance, BHCC will assist in scheduling an appointment with patient advocate.
- Attend and participate in staff meetings.
Minimum qualifications:
- Associate’s degree in human services or a Community Health Worker certificate, Employment history will be considered in lieu of degree.
- Preferred experience in behavioral health setting.
- Familiar with medical and pharmacological terminology.
- Experience working with various Microsoft Office applications—Word, Excel, Outlook, etc.
- Previous experience in an environment that provides direct patient care, case management, and/or patient education.
- Bilingual (English/Spanish) strongly preferred.
