Background
The Southside Coalition of Community Health Centers is a non-profit organization established to sustain, coordinate and improve health care access and delivery to the impoverished and vulnerable community members of South Los Angeles. We are a network of eight Federally Qualified Health Centers (FQHC) that are either located in South Los Angeles or serve a large portion of patients who live in South Los Angeles. Our collaboration creates collective strength that builds capacity, coordination and networking that focuses on the patient and supports quality, culturally and linguistically proficient delivery of care.
Position Summary
The Patient Navigator helps community clinic patients who have been hospitalized or treated in the Emergency Department at Dignity Health – California Hospital Medical Center (CHMC) schedule follow up medical appointments. The Patient Navigator meets with patients on-site at California Hospital Medical Center in downtown Los Angeles to assist and guide patients in receiving post hospital follow up appointments with their primary care provider. Applicants must be comfortable meeting with patients in hospital and emergency department settings. This is an onsite position with no remote work. Work hours: Monday – Friday 8:00 am – 4:30 pm. This position is a temporary two-year grant funded position.
Duties and Responsibilities
The Patient Navigator duties include:
- Meet with clinic patients who have been admitted to the hospital or are being treated in the emergency department. Navigator will meet with the patients on-site at the hospital.
- Completing general assessments to assess patient’s needs and connecting with appropriate resources and services as needed.
- Determine if the patient has established a “medical home” with one of our partnering clinics.
- Schedule follow-up appointments with patient’s clinic prior to or within 24 hours of discharge.
- Reporting challenges or barriers to care to patient’s clinic.
- Maintain confidentiality and ensure compliance with HIPAA regulations.
Minimum Qualifications and Experience Requirements
- Experience working with underserved populations in a community clinic, hospital, or social services setting a plus.
- Medical Assistant with 1-3 years of experience; Community Health Worker with 1-3 years of experience; high school/GED with 3-4 years of experience in a community clinic or hospital setting; Bachelor’s degree in social sciences field, health education or related field of study; Relevant experience considered will be inclusive of health education, public health, social work/case management or other related fields
- Good verbal and written communication skills to work effectively with people of diverse cultural, educational, economical, and social backgrounds.
- Basic computer and internet skills required for routine data collection, data entry and reporting experience with Microsoft Word and Excel Spreadsheets.
- Must pass background check and drug screening.

Pay: $23.00 - $24.00 per hour
