What are the responsibilities and job description for the Ryan White Peer Navigator position at Southern Tier AIDS Program Inc?
Southern Tier AIDS Program, Inc. is looking for a Ryan White Peer Navigator to join our Binghamton team! This position provides confidential supportive services to persons living with HIV/AIDS. This includes accompaniment services, serving as treatment resource, serving as agency and/or client liaison with health and human service providers and providing referrals, assistance with information and assistance with appointment scheduling. The Ryan White Peer Navigator reports directly to the Director of Programs, Intensive Services.
Responsibilities
Primary responsibilities include provision of accompaniment services to people living with HIV/AIDS, who receive services from STAP/STCC. This includes but is not limited to:
- engaging clients in the performance of: transportation services, reminder call services
- assisting with obtaining needed linkages and referrals, including: medical, legal, housing, social, financial, transportation, food security, and other requested services
- serving as client advocate, when necessary
- supplying client data, as needed, for reporting requirements, to STAP/STCC, NYS DOH, or other funding agencies
