Title: Early Intervention Physical Therapist, Contractor

Early Intervention (EI) builds upon and provides support and resources to assist family members and caregivers to enhance children’s learning and development through every day learning opportunities. Physical Therapist provides therapy to children birth to 3 in their natural environment.

Qualifications:

Master’s degree in physical therapy with license to practice independently; Preferred Qualifications: Doctoral degree in field; Education/experience working with children birth to age 3 and their families; Experience with working across early childhood systems Ability to work independently and in a team environment. Excellent interpersonal/written communication skills. Knowledge of community and social services resources helpful. Organized, detail oriented and able to multi-task. Computer skills required. Valid Ohio driver’s license, reliable transportation, and insurance.

Duties:

The contractor works with considerable independence, receiving general and professional direction from the Early Intervention Grant Coordinator. These activities may include, but are not limited to: Evaluation & Assessment of children and families seeking treatment, plans initial treatment, and interprets goals and treatment plan to parents and makes recommendations for further developmental assessments as needed; Participates as a member of interdisciplinary interagency evaluation team, interprets assessments results and determine eligibility for services; Maintains regular contact with and consults regularly with other professional staff; Prepares evaluations and progress reports at six month intervals, maintains client records; Participates in interdisciplinary interagency team to develop Individualized Family Services Plan; Implements IFSP for enrolled families, determines frequency and intensity to services based on physicians recommendation, child development status and family priorities; Assists with services coordination and assures utilization of other Community Services; Plans and implements treatment programs relating to early intervention therapy; Conducts therapy sessions in group and home visit settings for infants and toddlers with developmental disabilities. Maintains professional ethics in keeping with the confidentiality of information and material with which he/she will come in contact. Relates program philosophy, procedures and goals to the general public in a positive manner. Complies with Individual Bill of Rights. Maintains required certifications (First Aid/CPR) Works to provide a supportive, relationship based team environment. Maintains all documentation as it pertains to the Early Intervention COG grant requirements and deliverables Completes Early Intervention training requirements Working Conditions:

Working conditions may exist that are not such as normally exist in the occupation of public employees, which may include blood borne pathogens, communicable disease, potentially infectious materials, and/or aggressive behavior.

Working hours:

Up to 16 hours weekly, flexible schedule based on operational need. *This is a grant funded contract position.

Pay: $75.00 per hour
