This seamless care delivered locally by world-leading doctors gives patients what they need most when they are fighting cancer – peace of mind.About the Job : To assist patients, families, and caregivers with the psychological, social, emotional, spiritual, and practical issues associated with the diagnosis and treatment of cancer. To accompany patients through all phases of the disease continuum including diagnosis, treatment, life after cancer and supportive / palliative care. To provide oncology social work service to clients and their families.What You’ll Do : Assess new treatment patients to identify those with inadequate social and / or financial resources and caregiver support and then create meaningful intervention plans.Counsel patients and families on how to adjust to a cancer diagnosis and overcome emotional, social and psychological barriers to treatment plan adherence and to accessing community resources such as food, housing and transportation.Work in conjunction with the Financial Counselor to identify new treatment patients with insurance and / or financial challenges and create intervention plans.Participate in weekly multidisciplinary meeting by updating the Care Team on any psychosocial issues that might impact a patient’s adherence to their treatment plan.Work closely with providers to facilitate end-of-life discussions with patients.Who You Are : MSW Required.Experience in an oncology health care setting preferred.Epic experience, is a plus!Why You’ll Love Us : Sign-on bonus eligible position!Flexible day shift hours, no weekends.Health, dental, prescription, and vision coverage for full-time & part-time employeesCompetitive payTuition Reimbursement403(b) Retirement Savings PlanEducation & Paid training courses for continued career progression& So much more!#LAL
