What are the responsibilities and job description for the Community Health Worker II position at Southern Nevada Health District?
The Southern Nevada Health District (SNHD) announces an examination process to create an eligibility list to fill one (1) vacancy for the classification of Community Health Worker II in the Primary and Preventive Care and Community Health Divisions. The eligibility list may be utilized to fill vacancies in this classification District wide for up to four (4) months.
All new hires will have a starting salary of $22.74 per hour, which is non-negotiable.
The vacancy is funded by a grant and the continuation of the positions depend on the availability of grant funds and the business requirements of SNHD. If the funding is discontinued or becomes unavailable, your employment may be terminated without the right to appeal.
The current vacancy is in the Healthy Start Program, but the list may be used to fill future vacancies throughout the District.
This recruitment will close on or before the date list above, once a sufficient number of qualified applicants have been identified.
To provide services that increase health knowledge, self-sufficiency, and support self-management of diseases and health conditions to assigned clients/families through a range of activities such as outreach, health education, connecting individuals with resources, informal counseling, social/peer support and advocacy. To support providers, health educators, and case management teams through an integrated approach to care management and community outreach.

Education And Training
- Equivalent to graduation from high school.
- Minimum of one year of experience in community out-reach work
- Prior experience in medical record keeping desirable.
- Completion of Community Health Worker Certificate Program
- Possession of or ability to obtain and maintain a valid Nevada Driver's License as a condition of employment
- Prior experience in maternal child health preferred
- Prior home visiting experience preferred
