JOB SUMMARY:
Responsible for the overall day to day operations of the Shelby Co office and ensures that all care meets or exceeds compliance with state, federal and corporate guidelines.
QUALIFICATIONS:
1. Graduate of an accredited school of nursing.
2. Must be currently licensed (in good standing) in the state where the Hospice Program is located.
3. At least two years management experience in a hospice or related health care organization.
4. Knowledge of business management, federal and state regulations.
5. Speaks, writes, reads, and comprehends English.
6. Good communication and interpersonal skills.
RESPONSIBILITIES (including, but not limited to):
1. Evaluates goals and objectives for hospice services which meet and promote standards of quality care and meeting Unity’s Mission Statement, values and philosophy.
2. Maintains full responsibility of the day-to-day operations of the branch office.
3. Maintains compliance with federal and state regulations as appropriate.
4. Negotiates contracts and agreements for provision of services and assures enforcement of the agreements.
5. Acts as liaison to the Board of Directors.
6. Assists with orientation for new employees and in-services.
7. Maintains Quality Improvement Program.
8. Promotes hospice referrals in the community.
9. Supervise the planning, coordination, and safe delivery of patient/family care.
10. Assigns each patient/family to an appropriate staff member.
11. Plan and/or conduct IDG conferences. Present cases as indicated and participate in discussion, planning, re-evaluation, and updating patient Plan of Care.
12. Conducts supervisory visits with patient and staff as appropriate.
13. Audits medical records, checking for accuracy, documentation and appropriate patient care.
14. Evaluates performance of the staff.
15. Monitors continuous case situations to assure that at least nursing staff provides 51% of hands-on patient care.
16. Participates in community programs as requested, to promote professional growth and understanding of hospice care.
17. Provides the community with accurate information.
18. Initiates emergency procedures when necessary.
19. Communicates information effectively to appropriate personnel to facilitate continuity of care.
20. Accepts accountability for own practice.
21. Maintains competency by participating in in-services and continuing education programs.
22. Documents all patient related activities in a timely manner.
23. Reacts to change productively and performs other job-related tasks and duties as assigned.
24. Complies with accepted ethical conduct and professional Standards of Nursing Practice as set forth by the American Nurses Association.
25. Assesses patient’s condition and family needs through the collection of health data and the recognition of psychological, social, and spiritual factors that may influence the patient/family plan of care. Re-evaluates patient/family needs as required.
