What You’ll Do (a snapshot)
- Provide comprehensive wound assessment, treatment, documentation, and follow‑up for residents across cottage homes and the traditional building.
- Develop, implement, and evaluate individualized wound treatment plans in collaboration with physicians, wound specialists, CNAs, and families.
- Perform dressing changes, negative-pressure wound therapy support, debridement per protocol, and advanced topical applications as ordered.
- Monitor and report wound progress, trends, and potential complications; escalate per facility policy.
- Train and mentor nursing staff and CNAs on wound prevention, dressing technique, and documentation best practices.
- Participate in interdisciplinary team meetings, quality improvement, and infection prevention initiatives.
- Ensure accurate, timely EMR charting and compliance with regulatory standards.

Who We’re Looking For
- LPN or RN with current state license; wound care certification (WCC, CWCN, or similar) preferred but not required—willingness to pursue encouraged.
- Experience in wound care, long-term care, skilled nursing, or geriatrics is a plus.
- Strong assessment skills, critical thinking, and excellent communication with residents and families.
- Able to lift/move residents as needed and work across multiple small-home settings during a shift.
- Compassion, creativity, and a commitment to person-centered care.
Shift Options
- Days/Monday- Friday/ On-Call Rotation/ Willingness, as a Nurse Leader, to work as a charge nurse when necessary.
