What are the responsibilities and job description for the HOSPITAL LIAISON position at Southeast Healthcare?
Join our mission-driven team as a Hospital Liaison, serving as the bridge between inpatient psychiatric hospitals and community-based behavioral health services. In this role, you will advocate for individuals during hospitalization, complete assessments for new referrals, coordinate discharge planning, and ensure a smooth transition to ongoing care. Working collaboratively with hospital staff and Southeast Healthcare teams, you will help connect patients to the services and supports they need for successful recovery in the community.
Responsibilities:
- Serve as the primary liaison between Southeast Healthcare and private psychiatric hospitals to ensure continuity of care for patients.
- Conduct intake assessments and develop initial treatment plans for individuals referred for Southeast Healthcare services.
- Advocate for patients to ensure appropriate admissions, quality service delivery, and patient-centered care during hospitalization.
- Collaborate with hospital social workers, Southeast Healthcare case managers, and clinical teams to develop and coordinate safe, effective discharge plans.
- Facilitate connections between hospitalized patients and their assigned Southeast Healthcare treatment teams prior to discharge.
- Coordinate with the Intake Program Manager and Clinical Office Liaison to assign new patients to appropriate programs and services.
- Monitor hospitalized patients already receiving services and communicate updates that may impact treatment or discharge planning.
- Participate in hospital treatment team meetings as needed to advocate for patients and support collaborative care planning.
- Bachelor's degree in social work, Counseling, Psychology, or a related behavioral health field required; master's degree preferred.
- Current Ohio LSW or LPC license required; LISW or LPCC preferred.
- Experience providing behavioral health services and care coordination for individuals with serious and persistent mental illness (SPMI) or complex behavioral health needs.
- Working knowledge of Ohio mental health laws, patient rights, and behavioral health regulations.
- Valid Ohio driver's license, reliable transportation, and the ability to travel between hospital and community locations as needed.
