Position Title: SEAHEC Community Health Worker
Location: Graham or Greenlee County, AZ. Occasional travel to Cochise, Graham, Greenlee, Pima & Santa Cruz Counties.
COMMUNITY HEALTH WORKER – Program Manager
Frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The community health worker is also a key part in the dissemination of health education and resources to the community.
Responsibilities/duties:
· Community Health Workers (promotores de salud) will work and travel in Southeast border region communities to address health education following Community Health Worker core competencies:
o Health Education, Facilitation & Outreach: Provide culturally appropriate health education to the community. Through in person interactions, materials (in print and virtual) and follow up phone calls.
o Service Coordination: Works closely with CHW Supervisor and Program Manager.
o Communication: Communicate with community, CHW supervisor, Program Manager and others as needed.
o Relationship building: Become a trusted member of the community, interacting comfortably with community members and others on the team.
o Cultural Mediation
o Coaching & social support
o Community capacity building
o Direct service: education AND referral to resources
CORE SEAHEC JOB FUNCTIONS/COMPETENCIES:
· Support other SEAHEC projects and initiatives.
· Represent SEAHEC at meetings, events, schools and organizations.
· Participate and/or coordinate existing community health coalitions/activities.
· Work consistently toward SEAHEC mission, contributing to program and policy development.
· Collaborate with community agencies to provide health education, workshops and trainings.
· Collaborate in production of SEAHEC quarterly and annual reports including compilation of statistical and narrative program information under guidance of Executive Director.
· Other tasks as required by Executive Director.
REMOTE WORK REQUIREMENTS:
- Report to Program Manager on a weekly basis to review progress, discuss challenges, and receive guidance on tasks.
- Maintain an activity log documenting daily tasks, interactions, and outcomes. This log should be submitted to the Program Manager on a weekly basis for review.
- Ensure timely communication with the Program Manager and other team members via email, phone, or virtual platforms as necessary.
REQUIREMENTS:
· Able to lift 20lbs-30lbs
· Strong communication skills
· High School Diploma or equivalency
· Experience working with community members
· Bilingual English/Spanish oral and written proficiency
· Member of community or surrounding areas or knowledgeable of area
· Reliable transportation and willing to travel to border communities in Mexico as needed
· Experience in working with rural, border and/or other underserved communities.
Pay: $42,000.00 - $45,000.00 per year
