SWMCCC is dedicated to supporting individuals under community supervision, helping them achieve successful reintegration and promoting public safety. We are looking for a compassionate and organized Case Manager to join our Community Supervision Program.
Job Description:
As a Case Manager, you will work closely with clients in the community supervision program, providing guidance, support, and resources to help them meet their goals and comply with program requirements. You will serve as a key point of contact for clients, ensuring they have access to the services they need to succeed.
Responsibilities:
- Client Support:
- Assist clients in identifying needs and accessing community resources such as housing, employment, medical care, transportation, and social services.
- Provide guidance, encouragement, and ongoing check-ins to support clients in meeting their goals.
- Help client’s complete applications, paperwork, and referrals.
- Administrative & Secretarial Tasks:
- Answer phones, schedule appointments, manage calendars, and maintain organized records.
- Prepare and process documentation, reports, and correspondence.
- Maintain accurate client files (electronic and paper) in accordance with confidentiality standards.
- Insurance & Billing Support:
- Assist the Clinical Director with insurance billing tasks, including verifying coverage, gathering required documentation, and preparing billing submissions.
- Track authorizations, follow up on denials or missing information, and help ensure timely claim processing.
- Program Coordination:
- Support internal communication between staff, providers, and partnering agencies.
- Participate in team meetings and contribute to a positive, collaborative work environment.
Qualifications:
- Strong organizational and multitasking skills
- Excellent communication and customer-service abilities
- Basic knowledge of insurance processes (preferred, but training provided)
- Comfort working with diverse populations
- Proficiency with computers, scheduling systems, and standard office software
