What are the responsibilities and job description for the OASAS/SUD Case Coordinator position at South Shore Association for Independent Living?
Provide support and services to assist consumers with maintaining sobriety and psychiatric stability. Provide linkages for obtaining substance use and/or psychiatric treatment within a community environment.
Duties and Responsibilities
Encourages and supports consumers with sobriety, stability, and treatment linkages.
Coordinates services with Clinical Coordinator, Supervisor, and Care Managers.
Visits consumers at least twice monthly or as needed.
Teaches independent living skills to sustain housing and prevent homelessness.
Develops Individualized Recovery Plans and monitors progress on goals.
Ensures fire safety equipment is present and educates residents on use.
Teaches emergency protocols (medical, psychiatric, fire, food, etc.).
Acts as liaison with landlords, teams, and mandated treatment providers.
Completes timely documentation and attends required meetings.
Qualifications And Competencies
Bachelors degree in Human Services (BSW preferred).
3 years experience with target population.
Organizational, Communication, Critical Thinking, And Judgment Skills Required.
Physical Demands & Work Conditions
Valid drivers license and car with insurance.
Evening/weekend availability required.
Completion of mandatory trainings and fingerprinting.
Adheres to HIPAA and agency compliance plan.
