Position Overview
South River Federal Solutions LLC is seeking an experienced Registered Nurse to serve as the Lead Medical Case Manager and Contract Manager supporting the Vermont Army National Guard. This individual will provide direct medical case management services while also serving as the primary contractor point of contact responsible for oversight of contract performance and personnel. The position is designated as the sole Key Personnel role under the contract.
Primary Responsibilities
- Coordinate all assigned aspects of Service Member medical care and case management.
- Schedule and coordinate medical appointments and referrals.
- Manage and track Medical Evaluation Board (MEB) and Physical Evaluation Board (PEB) cases.
- Support medical readiness and deployability initiatives.
- Coordinate care among military, civilian, and specialty providers.
- Participate in Soldier Readiness Processing and other medical readiness events.
- Ensure compliance with HIPAA, Privacy Act, and applicable DoD requirements.
- Serve as Contract Manager responsible for oversight of personnel, performance, quality control, and communication with Government stakeholders.
- Respond to Government inquiries and resolve performance issues in coordination with the COR.
Minimum Qualifications
- Current, active Registered Nurse (RN) license in good standing in the State of Vermont.
- Minimum two (2) years of supervisory or team lead experience.
- Experience providing medical case management, care coordination, utilization management, or related nursing services.
- Experience working with military, VA, DoD, TRICARE, occupational health, disability evaluation, or complex care management programs preferred.
- Strong written and verbal communication skills.
- Ability to obtain and maintain required installation access credentials and background investigation requirements.
Desired Qualifications
- Certified Case Manager (CCM) or similar case management certification.
- Prior National Guard, Reserve, Military Health System (MHS), DHA, or TRICARE experience.
- Experience supporting MEB/PEB programs.
- Experience leading multidisciplinary healthcare teams.
Work Schedule
- On-site support Monday through Friday during Government operating hours.
- Occasional support for medical readiness events and Soldier Readiness Processing activities.
- Telework may be authorized only during approved Continuity of Operations (COOP) events.
Security Requirements
- Ability to successfully complete required background screening and installation access requirements.
- Completion of mandatory DoD and Army training requirements.
