We are looking for a compassionate and dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our nursing team. This position is ideal for a nurse who enjoys building relationships with residents, working as part of a supportive team, and providing high-quality care in a skilled nursing and rehabilitation setting.
As an RN/LPN, you will play an important role in ensuring our residents receive safe, personalized, and compassionate care while helping create a positive environment for residents, families, and staff.
Responsibilities
- Provide direct nursing care in accordance with resident care plans, physician orders, and facility policies.
- Assess and monitor residents for changes in condition and communicate concerns promptly.
- Administer medications and treatments accurately and safely.
- Complete nursing documentation and maintain accurate resident medical records.
- Coordinate care with physicians, CNAs, therapy, social services, and other members of the interdisciplinary team.
- Communicate with residents and families regarding care, treatments, and changes in condition.
- Provide wound care and other skilled nursing treatments as ordered.
- Respond appropriately to emergencies and changes in resident condition.
- Provide compassionate care to residents with dementia, cognitive impairment, and other complex healthcare needs.
- Supervise and support CNAs while promoting teamwork and quality resident care.
- Follow infection prevention, safety, and regulatory standards.
- Advocate for residents' dignity, preferences, independence, and quality of life.
Qualifications
- Current RN or LPN/LVN license in good standing.
- CPR/BLS certification preferred.
- Skilled nursing, long-term care, rehabilitation, or similar experience preferred, but new graduates are encouraged to apply.
- Strong communication and organizational skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Dependable, compassionate, and resident-focused.
- Ability to prioritize resident needs in a fast-paced healthcare environment.
