Job Overview

We're looking for a Community Health Worker (CHW) who cares about people and wants to make a difference in Grundy County.

As a Community Health Worker, you'll work one-on-one with people, walking alongside them to understand what they need and helping them navigate barriers to better health. A big part of this work is helping people get where they need to go - whether that's to the doctor, the grocery store, or other important places. You'll help make sure connections actually happen, whether that's getting connected with a healthcare provider, making it to appointments, or accessing food, housing, transportation, and other resources.

You'll spend time both at South Cumberland Community Clinic and out in the community, building relationships, supporting patients, and working alongside a team committed to improving health in Grundy County.

This position is part of the LIFT Grundy Health Mobility Project and includes a paid training program that helps you build skills and work toward becoming a certified Community Health Worker in Tennessee.

Position Summary

The Community Health Worker (CHW) supports individuals and families in improving their health by building relationships, sharing information, and connecting people to services and resources in their community.

CHWs are trusted community members who build relationships, connect people to resources, and help individuals navigate systems to support their health and well-being.

This role is part of the South Cumberland Health Mobility Project (LIFT Grundy) and includes working in both clinical and community settings. CHWs provide medical assistance to clinicians and support patients with navigating healthcare, accessing transportation, and connecting to resources such as food, housing, and other basic needs.

CHWs also participate in a registered apprenticeship program, gaining hands-on experience, completing training, and working toward certification as a Community Health Worker in Tennessee.

The ideal candidate will have a passion for the mission of the clinic to provide high quality medical care at no cost to patients, who are made up of people who are uninsured. Candidates should be comfortable working comfortably in a professional medical setting.

Essential Duties and Responsibilities

Community Engagement and Support

• Build trusting relationships with community members to provide support, encouragement, and guidance.
• Meet with individuals in clinic and community settings to understand needs and help identify next steps.
• Provide health education and share information on topics such as chronic disease prevention, healthy living, and mental health.
• Offer informal support, goal-setting, and encouragement to help individuals follow through on care and services.

Navigation and Resource Coordination

• Help individuals and families navigate healthcare systems, including scheduling and preparing for appointments.
• Connect individuals to resources such as transportation, food assistance, housing support, and financial assistance.
• Support coordination of transportation through the LIFT Grundy mobility network.
• Follow up with individuals to support referrals, appointments, and ongoing needs.

Clinical and Site-Based Support

• Support clinic operations and patient flow as needed, including welcoming patients and assisting with intake.
• Assist patients with completing forms and understanding next steps in their care.
• Take basic vital signs (such as height, weight, blood pressure, and pulse) as appropriate.
• Assist with basic clinic support tasks as needed, which may include preparing and handling lab samples (such as labeling, bagging, or processing samples) with appropriate training and supervision.
• Support communication between patients and providers, including follow-up on appointments, labs, prescriptions, and referrals.
• Provide additional administrative or front desk support when needed to ensure continuity of care.

Data Collection and Documentation

• Maintain accurate and timely records of client interactions, services provided, and outcomes.
• Enter and track data related to patient services, referrals, and program activities.
• Support tracking of program metrics, including appointments, services, and resource connections.

Program Participation and Team Collaboration

• Participate in building and strengthening a coordinated community mobility network.
• Collaborate with healthcare providers, social service agencies, and community partners.
• Attend regular team meetings, trainings, and check-ins with the CHW Coordinator.
• Work as part of a CHW cohort to share learning, support one another, and improve community health outcomes.

Position Details

Education

• High school diploma or equivalent required.

Experience

• Experience working with people in community, healthcare, or social service settings is preferred but not required.

Preferred Qualifications

• Connection to or lived experience in the South Cumberland Plateau community.
• Interest in healthcare, public health, or community-based work, with a passion for serving uninsured patients.
• Experience in customer service, caregiving, peer support, community outreach, or similar people-centered roles.

Work Environment

• Full-time position (40 hours/week).
• Typical schedule is 8:30 AM-4:30 PM on clinic days (Tuesday/Thursday) and 9:00 AM-5:00 PM on other weekdays.
• Work takes place in a combination of clinic, office, and community settings.
• Based in Tracy City, with regular travel throughout Grundy County and surrounding areas.
• Requires walking, standing, and occasional lifting of up to 25 pounds.
• This position is funded through June 2028.

Pay and Benefits

• Starting pay of $16.50 per hour.
• Planned pay increases to approximately $17.50/hour after six months and $18.50/hour after one year, based on successful completion of training and program milestones.
• 10 days of paid time off (PTO).
• 6 paid holidays after 90 days of employment.
• Mileage reimbursement for approved work-related travel.

Training and Professional Development

This position includes a paid Community Health Worker training program designed to help employees build the knowledge and skills needed for success. Employees receive:

• Paid on-the-job training.
• Ongoing supervision, coaching, and professional development.
• Support to complete Tennessee Community Health Worker certification.
• Opportunities to build skills in community health, patient navigation, healthcare teamwork, and community resource coordination.

Pay: $16.50 - $18.50 per hour
