INTRODUCTION:
The employee works under the supervision of the Director of Community Services. The employee will be expected to carry out goals and objectives as established in program plans and the essential job functions of the position. The employee is responsible for providing case management to help preserve, improve or create more desirable living conditions for the clients. The employee will work mainly indoors in an office environment but may be required to use own personal automobile to carry out a portion of his/her job duties (home delivery meals, and monitoring program activities). The employee may be exposed to foul odors, dust, dirt and tobacco smoke. The employee will be capable of operating a modern office equipment.
QUALIFICATIONS:
BS in Social Sciences/Work (Required)
Three (3) years’ relatable experience
Valid Tennessee Driver’s License
Proof of Automobile Liability Insurance
Satisfactory Background Check
WORKING CONDITIONS:
Flexible working hours (Normal work hours are 8:00 a.m. – 4:30 p.m., but this position may require some evening and weekend work hours). Normal sitting and movement in an office environment. May require occasional moving up to /-30 pounds, bending, stooping, or climbing. The work will be performed mainly indoors in an office environment, but there may be exposure to tobacco smoke, dirt, dust, and foul odors.
Requires travel to counties within the thirteen county service area for meetings and other job-related activities. Will require some travel for training outside the thirteen county area and occasional overnight travel.
ESSENTIAL JOB DUTIES:
Assume responsibility for overall operation of Case Management responsibilities
Provide oversight, support and assistance to Client Advocates in all aspects of Case Management
Network with other service agencies to secure and maintain partnerships for referral of clients.
Ensure that all client information is entered accurately into the database for tracking and reporting purposes
Prepare and/or review case management files for accuracy
Assist with taking applications, assessing and monitoring clients
Assist staff and/or clients with issues and concerns regarding case management
Ensure that all policies and procedures of Case Management are followed and in compliance with federal, state and local regulations of the Community Services Block Grant (CSBG)
Travel to all 13 NSC offices, and out of area for training when necessary
Assists with conducting and/or participate in training sessions for staff
Participates in various cross training efforts to assist with other Community Service Department functions
Provide coverage in NSC offices when necessary
Performs other related duties as requested
REQUIRED KNOWLEDGE AND ABILITIES:
Strong organizational, interpersonal and decision-making skills
Ability to deliver presentations and provide training in group settings
Ability to accurately handle detailed, confidential, and complex information
Capable of interpreting regulations and operational policies and procedures to ensure program compliance
Ability to prepare and maintain program files and related documentation in an organized and accessible manner
Knowledge of computer database applications including word processing and spreadsheet applications, and database management
While performing the duties of this job, the employee is frequently required to sit; stand; stoop; bend; walk; talk; hear; use hands to grip; reach with hands and arms, required to stand or walk on uneven surfaces; climb stairs and lift up to 30lbs daily.
