What are the responsibilities and job description for the Adult In Home Care Coordinator position at South Central Alabama Mental Health Center?

Position Overview: We are seeking a dedicated and passionate In Home Adult Care Coordinator to join our team in Butler/Crenshaw County. In this role, you will conduct home visits, obtain relevant information from adults with mental health illnesses in obtaining the services they are entitled to receive.

Qualifications:

  • Must have a Bachelor's degree in a mental health-related field.
  • Case Management training preferred.

Responsibilities:

  • Conduct home visits and obtain relevant information from adults with mental health illnesses.
  • Write plans of care tailored to the needs of each individual.
  • Establish liaisons with other service agencies in the community.
  • Assist individuals in obtaining the services they are entitled to receive.
  • Collaborate with other professionals to ensure comprehensive care for individuals.

ALL candidates must be able to type and have basic computer skills, possess a valid driver's license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
