Career opportunities abound at SOUND! SOUND Behavioral Health is growing, and we are hiring a Veteran Health Care Navigator supporting our Auburn, WA CCBHC clinic! 

 

The Health Care Navigator will act as a liaison between Sound's SSVF program and the VA or community medical clinic, and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.

Health Care Navigators provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. SSVF Health Care Navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties.

 

Location: 4238 Auburn Way N, Auburn, WA 98002.

Schedule: Monday-Friday 8:00am-5:00pm.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Case Management
  • Develops a care plan in collaboration with the Veteran and, whenever possible, the interdisciplinary treatment team, family members, and significant others. 
  • Integrates the Veteran's cultural values and preferences into the care plan, assists the Veteran in identifying methods to monitor progress toward meeting health goals, and provides ongoing follow-up.
  • Regularly reviews care plan goals with the Veteran, conducts non-clinical barrier assessments, and provides resources and referrals needed to support achieving outcomes.
  • Determines the needs, strengths, limitations, and preferences of each Veteran and will engage in problem-solving to identify and reduce barriers to care.
  • Coordinates referrals to VA, community health clinics, and other programs needed to ensure access to health care, based on the Veteran's preference.
  • Assists the Veteran in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the Veteran's care.
  • Reiterates provider recommendations using clear language to support the Veteran and family members or caregivers. 
  • Assists Veterans in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team.
  • Collaborates with community providers to maximize the use of VA and community resources.
  • Serves as the liaison to VA and community health care programs, and represents the program in contacts with other agencies and the public.
  • Serves as the subject matter expert on community health resources and collaborates with other providers in the ongoing reassessment of the Veteran's health care needs.
  • Coordinates supportive and additional services with the Veteran, in collaboration with the SSVF Case Manager.
  • Other duties as assigned.
  • The health care navigator will adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies (e.g., critical incident reporting, HIPAA, Duty to Warn).

Establishes and maintains positive, ethical, and professional working relationships with clinical team members, clients, and outside organizations. Relates to a wide variety of people, including members of ethnic and sexual minority populations, and is respectful of individual and cultural differences.

Qualifications

EDUCATION AND EXPERIENCE

  • Two years of experience in a healthcare or social services area of practice preferred. 
  • Bachelor's in social services or equivalent education and experience required. 
  • Experience supporting Veteran populations is preferred. 

KNOWLEDGE, ABILITIES, AND SKILLS

  • Possesses excellent judgment, effective communication, and strong collaboration skills.
  • Understands recovery concepts in behavioral health; synthesizes and applies, as appropriate to their work at Sound.
  • Ability to learn new tasks and skills quickly, and to manage and prioritize multiple tasks, while simultaneously managing crises.
  • Possesses strong working knowledge of the physical and/or behavioral health and support resources of King County, and is familiar with state and local health delivery systems, including knowledge of various funding principles and clinical services mandates.
  • Able to demonstrate the capacity to appreciate and work well with the complex and diverse population.

CERTIFICATES, LICENSES & REGISTRATIONS

  • A Washington State driver's license and an insurable driving record are required. This position does outreach in the community and travels to clients. 
  • Agency Affiliated Counselor Registration required upon hire.
  • Must pass a criminal background check. 

PHYSICAL DEMANDS

 

To successfully perform the essential duties of this position, an individual must be able to sit at a desk for four or more hours a day and use office equipment, including phones and computer keyboards, for four or more hours per day. Individuals must be able to engage vendors, team members, clients and others over the phone and in person by voice. Regular attendance is an essential function of the position. Bachelor's degree starting compensation range: $30.84-$32.41 per hour.

Master's degree starting compensation range: $31.90-$34.65 per hour.
