PCAP Case Manager
The Parent-Child Assistance Program (PCAP) is a 3-year intervention for pregnant and parenting mothers with substance use disorders and their families. This position is responsible for providing direct outreach, home visitation, case management, and advocacy services to mothers with at-risk alcohol or drug use during pregnancy, as well as their children, partners, and extended families when needed.
Job Duties
- Perform case management activities in compliance with PCAP policies and procedures, ensuring confidentiality and professionalism.
- Manage a caseload to serve clients and their families.
- Conduct home visits approximately twice per month, based on client needs.
- Assist clients in setting goals based on their individual needs assessments and develop steps to achieve those goals.
- Monitor client progress and adjust plans as needed.
- Identify and connect clients with high-quality community providers and agencies.
- Provide outreach services to engage clients with community agencies that align with their needs and goals.
- Administer clinical assessments in a professional manner, handling sensitive and confidential information.
- Collect client data and enter it into an online data entry platform.
- Coordinate scheduling and attend case consultations, appointments, and meetings with community providers.
- Maintain regular contact with clients' networks of service providers.
- Attend weekly individual supervision sessions with site clinical supervisor to discuss client needs and goals.
- Participate in weekly team meetings to evaluate client and program progress.
- Provide outreach services to locate missing clients and re-engage them in the program.
- Participate in required agency meetings and ongoing training.
- Contribute to report writing, training sessions, and community presentations as needed.
- Work effectively and independently in the field with minimal supervision, while effectively managing time, prioritizing tasks, and problem-solving in crisis situations.
PCAP Case Manager Education and Experience
- 4-year B.A. or B.S. degree and one year of community-based experience working with vulnerable populations
- Experience with issues/on projects working with prenatal substance use or equivalent combination of education and work experience
Standard Agency Requirements
- Adhere to HIPAA compliance, non-discrimination policies, and engage discreetly with high-risk communities and their families
- Have a valid driver's license and be able to travel within the service area daily and pass a driver’s abstract
- Provide copy of valid car insurance
- Pass criminal background and drug screening
- Proof of Covid vaccination or approved exemption
- Obtain and maintain current CPR, Infant CPR, and First Aid Certification
