Job Summary

Duties
- Assist residents or clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Monitor patient health by observing vital signs, noting changes in condition, and reporting concerns promptly
- Support medication administration in accordance with care plans and HIPAA regulations to ensure privacy and safety
- Prepare nutritious meals tailored to dietary needs and assist with meal consumption as needed
- Maintain cleanliness of living spaces by performing light cleaning duties to promote hygiene and comfort
- Use specialized equipment such as Hoyer lifts or resident’s personal assistive devices to ensure safe transfers and mobility as needed
- Document care activities accurately in compliance with care plans and regulatory standards like HIPAA
Qualifications
- Previous experience working in assisted living facilities, nursing homes, group homes, or home health environments preferred
- Knowledge of patient observation techniques, dementia care (including Alzheimer’s), memory care, and hospice care practices
- Strong communication skills to collaborate effectively with team members, residents, families, and healthcare professionals
- Compassionate attitude with a genuine desire to improve the lives of others through direct support and personalized care
