Home Health Registered Nurse (RN)
Job Summary
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes in accordance with physician orders and the established plan of care. The RN is responsible for performing comprehensive assessments, developing and implementing patient care plans, coordinating care with other healthcare professionals, and ensuring high-quality patient outcomes while maintaining compliance with Medicare and agency regulations.
Key ResponsibilitiesPatient Assessment
- Perform comprehensive patient assessments, including Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
- Complete and submit OASIS documentation accurately and timely
- Evaluate patient condition and identify nursing care needs
Skilled Nursing Care
- Provide skilled nursing services including:
- Wound care
- Medication management
- IV therapy
- Disease management
- Patient and caregiver education
- Monitor patient progress and report significant changes to the physician and Director of Nursing
Care Planning
- Develop and implement individualized Plan of Care (POC) in coordination with the physician
- Coordinate care with therapists, home health aides, and other members of the healthcare team
- Update care plans based on patient progress and clinical findings
Documentation
- Maintain accurate and timely clinical documentation in the agency EMR system
- Ensure documentation supports medical necessity and homebound status
- Submit completed visit notes within required timeframes
Communication
- Communicate effectively with physicians, patients, families, and interdisciplinary team members
- Participate in case conferences and care coordination meetings
- Report patient concerns and changes in condition promptly
Compliance
- Follow Medicare Conditions of Participation (CoPs) and agency policies
- Maintain patient confidentiality and comply with HIPAA regulations
- Adhere to infection control and safety protocols
QualificationsRequired
- Current Registered Nurse (RN) license in the State of California
- Minimum 1 year clinical nursing experience
- Valid driver's license and reliable transportation
- Current BLS certification
Preferred
- Home Health experience
- Experience with OASIS documentation
- Familiarity with home health EMR systems (Axxess, WellSky, Data Soft Logic)
Pay: $100.00 - $150.00 per hour
Work Location: In person
