Sonoma Valley Community Health Center (SVCHC) is a non-profit, federally qualified health center (FQHC) that opened in 1992 as a free-standing primary health care clinic. SVCHC began as a grassroots effort to address the lack of access and disparities of health care for the uninsured and underserved in Sonoma Valley. Since its inception, we have grown in capacity to serve Medi-Cal, insured, underinsured, private insurance, private pay patients, and provide sliding scale fees for those in need. In addition to medical services, we offer behavioral health, an in-house phlebotomy service, dental clinic, optometry clinic, pharmacy services, and certified bilingual enrollment professionals to help determine eligibility and enrollment options. We believe in providing access to quality healthcare to all of our neighbors in Sonoma Valley.

A Community Resource Specialist (CRS) with SVCHC provides patients with comprehensive clinical resources and non-clinical resources and works to ensure better health outcomes for the patient. This position collaborates with other clinical staff to assess patient needs and coordinate services. CRS team members provide resources and support to at-risk patients and coordinates necessary specialty referrals. This position is also responsible for tracking and reporting activities related to resources, support and referrals. Community Resource Specialists work to maximize the financial benefits of all patients. The position assists patients wanting to establish care as well as pursuing enrollment in government programs such as Medi-Cal Assistance, Enhanced Care Management, Cal-Fresh, and Covered California.

JOB DUTIES INCLUDE:

  • Works with patients with complex social issues to identify appropriate resources. Patient population may include individuals dealing with domestic violence, poverty, chronic mental illness, homelessness or substance abuse.
  • Create and maintain care plans on all ECM enrolled patients.
  • Provide resources, support, outreach and coordinated care to SVCHC patients.
  • Facilitates enrollment and appointments for specialty and voucher diagnostic care.
  • Manages tracking of utilization resources.
  • Screens patients for eligibility and assists those who are applying for government programs.
  • Follows up on patient applications to ensure work is done completely and accurately.
  • Maintains expertise on various benefit resources that may aid patients such as Medi-Cal, Medicare and other public programs.
  • Interprets and obtains understanding of patient coverage information.
  • Receives patient referrals from staff and interfaces regularly with medical, behavioral health, optometry and dental providers and support staff to provide patient assistance.
  • Demonstrate cultural competence and conflict resolution skills.
  • Works with agencies, providers and clinical staff to trouble shoot problems in communication regarding various programs and services available internal and external.

QUALIFICATIONS

*Community Health Worker Certificate required
*Bilingual in English and Spanish

*Education:Associate degree in health care, social services or related field or high school diploma or equivalent with at least two (2) years health care/ social service experience. Bachelor's Degree preferred.

*Experience:Minimum two years of experience working in a community-based setting, preferably in a Non-profit setting.

Pay: $26.00 - $34.00 per hour
