We are seeking a dedicated and compassionate Endocrinologist to join our growing team and provide specialized care to patients with endocrine and metabolic conditions. Position Summary

The Endocrinologist will evaluate, diagnose, treat, and manage patients with a variety of endocrine disorders, including diabetes, thyroid disease, metabolic conditions, and hormonal imbalances. The ideal candidate will collaborate with primary care providers and interdisciplinary care teams to ensure coordinated, comprehensive, and high-quality patient care.

Key Responsibilities

• Evaluate, diagnose, and manage patients with endocrine and metabolic disorders, including diabetes, thyroid conditions, adrenal and pituitary disorders, and other hormonal imbalances.
• Develop individualized treatment plans based on patients’ clinical needs, medical histories, and treatment goals.
• Provide diabetes management, metabolic risk reduction, and patient education to support long-term health outcomes.
• Collaborate with primary care physicians, nurse practitioners, physician assistants, and other specialists to coordinate care and facilitate appropriate referrals.
• Monitor treatment effectiveness, adjust medications, and order and interpret relevant diagnostic tests.
• Maintain accurate, timely, and comprehensive documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and support evidence-based clinical practices.
• Actively participate in SOMOS Medical PC’s Patient-Centered Medical Home (PCMH) model by supporting care coordination, patient engagement, preventive care, and chronic disease management.
• Communicate effectively with patients and families using culturally and linguistically appropriate approaches.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
• Completion of an accredited Internal Medicine residency and Endocrinology fellowship.
• Board-certified or board-eligible in Endocrinology.
• Active and unrestricted New York State medical license.
• Strong clinical judgment, communication, and interpersonal skills.
• Commitment to providing compassionate, culturally responsive, and patient-centered care.
• Ability to work collaboratively within a multidisciplinary healthcare environment.
