Key Responsibilities
- Conduct patient assessments to identify social service needs.
- Develop and implement personalized care plans in collaboration with an interdisciplinary team.
- Provide emotional support and counseling to patients and families.
- Advocate for patient needs within the facility and coordinate with external resources.
- Maintain accurate and confidential patient records.
- Oversee discharge planning and ensure continuity of care post-discharge.
Qualifications
- Minimum 1 year of experience in Social Work or a related field.
- Masters in Social Work
- Ability to meet state requirements of 120 plus bed facility
- Experience in a long-term care or healthcare setting, particularly in social services.
- Strong understanding of case management and care coordination.
- Exceptional communication and interpersonal skills.
- Ability to work compassionately with a diverse patient population.
