As a Care Manager, you’ll play a hands-on role in helping older adults and adults with physical disabilities live safely, independently, with dignity and choice. You’ll work directly with individuals aged 60 and adults 18 with physical disabilities—along with their caregivers—to identify needs, navigate available resources, and connect them to the right supports at the right time.
This is a client-facing role for someone who values relationship-based work, thoughtful assessment, and practical problem-solving.
What you’ll do:
- Conduct intake interviews with clients and caregivers, by phone and in person, to understand needs and identify appropriate services and referrals.
- Complete comprehensive in-home assessments for frail and vulnerable adults, evaluating:
o Living environment and safety
o Functional and psychosocial status
o Support systems, cultural considerations, and financial factors
- Partner with clients and caregivers to link them to community-based services and resources that support independence and quality of life
- Visit county senior centers and assess participants to determine appropriateness for center-based programming
- Serve as a trusted point of contact for clients and families navigating complex systems

Essential Functions
- Learns and remains current with service standards and entitlement programs like Jersey Assistance for Community Caregivers (JACC), Managed Long Term Support Services (MLTSS), Medicaid programs, and other benefit programs (i.e., PAAD, Senior Gold and Medicare Part D).
- Provides direct guidance and assistance with entitlements.
- Develops, implements, and evaluates care plans for clients and caregivers.
- Performs all visits and contacts per program guidelines.
- Keeps abreast of changes in the field of gerontology/disability and participates in job-related educational programs.
- Develops and maintains service resources.
Qualifications
Essential
- Bachelor’s degree in social work from an accredited college or university
- Ability to engage and develop rapport with clients.
- Strong analytical, communication (written and oral) and interpersonal skills.
- Minimum one year experience in working with older adults (age 60 ) and/or adults (age 18 ) with physical disabilities in an institutional or community setting. Other relevant direct service experience may be considered.
- Demonstrated experience in assessing older adults (60 ), and adults with physical disabilities. Other relevant experience assessing adults may be considered.
- Ability to problem-solve, including but not limited to, problem solving specific to fieldwork, time management and caseload management.
- Ability to conduct comprehensive assessments; develop, implement, and revise service plans.
- Ability to independently commute between office, client/caregiver locations and meeting sites.
- Ability to read, write and communicate clearly in English.
- Valid New Jersey driver’s license.
- Proficiency in the use of computers, and standard business software.
Preferred
- Current knowledge of benefit and entitlement program eligibility criteria. If not, will be supported with on-the-job training.
- State Health Insurance Program (SHIP) and Association of Information Services (AIRS) certification. If not certified, will be supported to receive certification while on the job.

Salary / Pay Rate: $48,125-$54,000/year for 35 hours/week
