Solvara Partner Services, Inc. is a locally owned and therapist-operated rehabilitation company looking for Physical Therapists to join our growing team throughout Hartford County.

We're not a large corporate home-health organization. We're practicing therapists who understand what it's like to manage a busy caseload, travel between patients, and balance documentation with everyday life.

We believe therapists should have a say in their schedules, work within a reasonable geographic area, and have direct access to the people running the company. No regional managers, no complicated chain of command, and no feeling like you're just another number.

You do not need to cover the entire county. We're happy to work with therapists who prefer a smaller, more manageable territory.

• Perform physical therapy evaluations and reassessments in patients' homes

• Develop individualized treatment plans based on patients' functional needs

• Provide skilled interventions addressing mobility, strength, balance, gait, transfers, and fall prevention

• Educate patients and caregivers on safety, mobility, and home exercise programs

• Complete accurate and timely clinical documentation

• Communicate with other healthcare professionals involved in patient care

• Active Connecticut Physical Therapist license

• Degree from an accredited Physical Therapy program

• Strong clinical assessment and communication skills

• Ability to work independently in a home-care setting

• Reliable transportation

• Previous home-care experience preferred, but not required
