Role Description This is a full-time, on-site Home Care Provider role based in Brooklyn, NY. The Home Care Provider will support clients with daily living activities such as bathing, dressing, grooming, toileting, and mobility assistance, while promoting comfort and safety in the home. Responsibilities include assisting with meal preparation, light housekeeping, medication reminders (as permitted), and accompanying clients to appointments or errands as needed. The role also involves monitoring client well-being, observing and reporting changes in condition, and following individualized care plans. The Home Care Provider will build trusting relationships with clients and families, maintain clear documentation of services provided, and collaborate with supervisors to ensure high-quality, person-centered care.
Qualifications
- Strong caregiving skills, including experience supporting individuals with daily living activities and fostering client dignity and independence.
- Hands-on home care experience, with the ability to maintain a safe, clean, and organized environment in a client’s residence.
- Personal care capabilities, such as assistance with bathing, dressing, grooming, toileting, and mobility, delivered with respect and discretion.
- Elder care experience, including familiarity with age-related needs, safety considerations, and compassionate support for older adults.
- Excellent interpersonal skills, including empathy, patience, active listening, and clear communication with clients, families, and colleagues.
- High school diploma or equivalent; relevant certifications (e.g., HHA, PCA, CNA) are preferred where applicable.
- Ability to lift, transfer, and assist clients safely, and to perform physical tasks required in home care settings.
- Reliable transportation to client homes in Cleveland, OH and willingness to work a full-time schedule, including possible evenings or weekends.
- Background check and reference verification required; current CPR/First Aid certification is an advantage.
