Job Summary
Join our compassionate healthcare team as a Hospice Registered Nurse (RN), where you will play a vital role in delivering high-quality, patient-centered end-of-life care. Responsibilities

  • Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual needs, ensuring personalized care plans are developed and implemented effectively.
  • Administer medications including pain management therapies and other treatments as indicated for symptom relief.
  • Provide wound care while maintaining strict aseptic techniques to prevent infections.
  • Educate patients and families on disease progression, medication use, advanced directives, and comfort measures to promote informed decision-making.
  • Coordinate interdisciplinary care by collaborating with physicians, social workers, therapists, and other healthcare professionals to optimize patient outcomes.
  • Document all patient interactions accurately using EMR (Electronic Medical Records) systems while adhering to HIPAA regulations.
  • Respond promptly to triage calls and urgent situations in the community setting, providing guidance on symptom management or emergency interventions as needed.

Skills

  • Must be able to maintain excellent communication and positive interactions, including with co-workers, patients, families, facility partners, etc.
  • Proven experience in hospice & palliative care with strong knowledge of pain management and symptom control strategies.
  • Critical care experience including ICU or emergency medicine enhances ability to handle complex cases with confidence.
  • Experience working with diverse populations including geriatrics, patients with developmental disabilities, memory care needs, or those in assisted living or long-term care settings.
  • Strong understanding of infection control practices and aseptic techniques essential for safe patient care.
  • Ability to perform physical examinations, vital signs assessment, medication reconciliation, and diagnostic evaluation in home or community environments.
  • Excellent communication skills for teaching families about disease processes and providing emotional support during difficult times.
  • Certification in Basic Life Support (BLS) required.

Pay: $49.00 - $54.00 per hour

Expected hours: 1 – 24 per week
