Responsibilities
- Develop personalized support plans tailored to each client’s unique needs and goals
- Coordinate services across multiple agencies, including healthcare providers, social services, and community organizations
- Assist clients with navigating complex systems such as Medicaid, Section 8 housing, child welfare programs, and workforce development initiatives
- Provide crisis intervention and crisis management support during urgent situations to ensure client safety and stability
- Utilize motivational interviewing techniques to encourage positive behavioral changes and foster client engagement
- Maintain detailed documentation of client interactions, service plans, and progress notes in compliance with organizational standards
Qualifications
- Proven experience in social work, behavioral health, or related fields with a focus on client advocacy and support coordination
- Strong skills in crisis intervention and crisis management to handle urgent situations effectively
- Working knowledge with OCDD Medicaid Waivers and OAAS Targeted Populations
- Experience supervising staff or mentoring peers is preferred
- Excellent communication skills with the ability to motivate and empower clients through motivational interviewing techniques
