Overview
The Rehabilitation Specialist is responsible for overseeing and coordinating comprehensive rehabilitation services within our Psychiatric Rehabilitation Program (PRP). This role includes organizing program services to facilitate individuals’ recovery, decision-making, and integration within home, school, work, and community settings. Key responsibilities include developing individualized rehabilitation plans, conducting assessments and screenings, and ensuring the delivery of quality, person-centered treatment through supervision and support of PRP staff. The Rehab Specialist will provide training and guidance to staff, manage documentation and compliance in line with regulatory standards, and maintain interdisciplinary collaboration to meet client needs.
Key Duties & Responsibilities
Client Assessments and Treatment Planning:
- Conduct intake assessments, develop Individual Rehabilitation Plans (IRP), and conduct necessary screenings to establish personalized treatment goals.
- Ensure that all PRP services are documented accurately and align with client-centered goals.
Staff Orientation, Training, and Supervision:
- Provide orientation, supervision, and ongoing training for PRP staff to reinforce best practices in PRP service delivery, documentation standards, and client-centered care.
- Conduct individual and group supervision sessions to support staff development and reinforce adherence to agency protocols and clinical standards.
Quality Assurance and Compliance:
- Review and approve client records, documentation, and progress notes to ensure compliance with agency policies, Medicaid regulations, CARF standards, and licensing requirements.
- Enforce and maintain adherence to electronic documentation entry and approval timelines as required by the agency.
Direct Client Support and Crisis Intervention:
- Facilitate group sessions, crisis intervention, and therapeutic support to address immediate client needs and promote positive mental health outcomes.
- Identify and develop group curriculums tailored to the demographic and clinical needs of clients, supporting their social, emotional, and vocational development.
Interdisciplinary Collaboration and Holistic Care:
- Collaborate closely with interdisciplinary teams, including behavioral health providers, primary care staff, and external agencies, to ensure integrated and holistic client care.
- Maintain open communication with C-Suite staff to relay pertinent clinical and administrative issues impacting program success.
Policy Enforcement and Regulatory Compliance:
- Ensure staff compliance with company policies and state regulations, addressing disciplinary issues in line with agency protocols when necessary.
- Monitor client satisfaction, assess the quality of services provided by direct care staff, and ensure strong clinical and rehabilitative skills are consistently applied.
Program Improvement and Reporting:
- Prepare and submit regular program updates, including staff and regulatory concerns to the Program Director, and participate in quality improvement efforts.
- Document all planned and unplanned discharge summaries within 15 days of discharge, ensuring timely and comprehensive client follow-up.
Additional Responsibilities:
- Develop and implement structured group activities and curriculums that align with the clients’ clinical profiles and rehabilitation goals.
- Monitor and maintain comprehensive records for client satisfaction and outcomes to ensure continuous program quality and alignment with agency objectives.
Minimum Qualifications:
- Must be a licensed Maryland mental health professional (LMSW, LCSW-C, LGPC, LCPC, LGMFT, LCMFT) OR have a certification from the Commission of Rehab Counseling (CRC)
- Must be able to pass background checks as required by facility policies
- Strong communication skills, both verbal and written
- Sensitivity to and understanding of cultural diversity and special needs populations
- Valid Driver's License and clear driving record
- Supervisory experience is a plus
- Strong knowledge of state and regulatory guidelines for psychiatric rehabilitation services (COMAR regulations)
- Proficiency in clinical documentation and treatment planning
- Ability to work collaboratively with multidisciplinary teams and external agencies
