LVN Field Nurse
We are seeking a compassionate Licensed Vocational Nurse to join our care team, who's responsibility is to provide quality care to our patients. Assessing patient's medical conditions, maintaining patient's comfort, meeting the needs of individuals within their home environments, along with providing their loved ones with guidance and support.

LVN Field Nurse
We are seeking a compassionate Licensed Vocational Nurse to join our care team, who's responsibility is to provide quality care to our patients. Assessing patient's medical conditions, maintaining patient's comfort, meeting the needs of individuals within their home environments, along with providing their loved ones with guidance and support.
Responsibilities:
- Under the direction of a Registered Nurse, the Licensed Vocational Nurse is responsible for providing skilled nursing and direct patient care to assigned.
- Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team
- Conduct home visits for patients (Travel required for patient visits).
- Ability to work with terminally ill patients and their families
- Demonstrates competency in performance of skilled nursing functions
- Good communication with physician, caregivers, family, and hospice team members
- Uses equipment and supplies effectively and efficiently
- Provide accurate and maintains timely patient documentation (Charting)
- Provides appropriate pain/symptom management
- Evaluates patient’s response to treatments/medications
- Performs job compliance with agency policies, procedures and applicable laws
- Participates in quality assessment performance improvement teams and activities, including professional growth and development
- Must have valid/current California Licensed Vocational Nursing License
- Must have at least 1 year of recent LVN experience in home health, hospice, palliative care, acute care &/or skilled nursing
- Current CPR/BLS
- Current CA driver’s license with proof of insurance, reliable transportation and willingness to drive to patient's location
- Must have compassion, patience, be honest and dependable
Salary : $40 - $45
