Providing physical therapy services in an outpatient setting, this contract position is based in Payson, AZ. The role involves evaluating assigned patients, creating and adjusting treatment plans aligned with patient needs, goals, and physician directives. The therapist will oversee discipline-specific therapy services for assigned patients, ensuring functional, community-focused treatment programs. Active participation in patient, family, student, and coworker education is required, along with maintaining professional standards and compliance with organizational codes of conduct.

Key Responsibilities

• Conduct comprehensive patient evaluations and develop goal-oriented treatment plans.
• Delegate and direct therapy services for assigned patients.
• Document medical records accurately, including goal setting, quality assessments, and letters of medical necessity.
• Ensure treatments comply with physician orders and insurance approvals.
