A home infusion specialty pharmacy is seeking 3-5 Patient Care Coordinators (PCCs) to join their New Castle, DE location on a contract to hire basis. This role focuses on acting as the key liaison between the pharmacy, patients, healthcare providers, and caregivers to facilitate upcoming medication orders and support ongoing patient care.
Work Schedule
- 8:30 a.m. to 4:30 p.m. and/or 9 a.m. to 5 p.m.
- Monday through Friday
- Facilitate communication between patients, providers, caregivers, and the pharmacy department to ensure timely and accurate medication order processing
- Conduct patient assessments with each order refill, including dosage calculations and clinical evaluations
- Manage insurance verifications and prior authorizations to support medication access
- Maintain meticulous and organized workflow to meet daily operational metrics
- Collaborate effectively within a focused team environment while demonstrating self-motivation and accountability
- Minimum 1-2 years of pharmacy technician experience, preferably in retail or long-term care settings
- Strong knowledge of pharmacy processes including dose calculations and clinical basics
- Familiarity with insurance procedures and prior authorization requirements
- Experience with CareTend software is preferred but not required
- High School Diploma or GED required; national pharmacy technician certification highly preferred
- Excellent communication skills and reliability are essential
