Location: Berkeley, CA Sign-On Bonus: $10,000 Position: Pharmacy Specialist Duties:Initiate, modify, and discontinue medications within a Collaborative Practice Agreement Focus on management of plex disease states to help patients reach optimal therapeutic goals Simplify patient medication regimens, minimize the risk of harmful medication interactions, and educate patients to enhance adherence and clinical outes Responsible for policy development, continuous process improvement, continuing education, clinical and medical staff development, and Pharmacy APPE student and resident preceptorship Perform patient care assessments, order and interpret laboratory tests, prescribe, modify, and discontinue medication therapies Monitor clinical outes, municate with medical providers, perform electronic documentation, and refer patients to other healthcare providers Schedule: Monday to Friday, day shifts Ideal Candidate:PharmD PGYI PGYII or 4 years of current ambulatory care oncology experience BCOP preferred Ability to obtain CA pharmacy license Contact Information: If interested, please contact Thomas Tatham at thomas.tatham@soliant. or call/text .
