POSITION SUMMARY:

The Case Manager is responsible for providing trauma-informed, culturally responsive support, crisis intervention, and resource navigation for individuals and families impacted by community violence.

DUTIES AND RESPONSIBILITIES:

  • Trauma-Informed Assessment by conducting strength-based assessments addressing histories of community trauma, violence exposure, and co-occurring challenges.

  • Trauma-informed care by building trusting relationships honoring cultural humility and psychological safety.

  • Care Coordination by linking clients to wrap-around resources, including mental health counseling, substance use support, legal assistance, housing aid, tattoo removal, employment readiness, etc. 

  • Service Planning by developing individualized service plans focused on safety, behavioral health, stable housing, and educational and employment goals.

  • Community Outreach by collaborating with multidisciplinary partners including trauma centers, law enforcement liaisons, local intervention workers, and community organizations.

  • Crisis Intervention by proving immediate de-escalation, risk assessment and safety planning during critical incidents.

  • Documentation and Compliance support by maintaining accurate, timely case notes and tracking client activities and outcomes, community engagement and other data collection required by the contract for service delivery.  

  • Prepare monthly case-related reports including outcomes, successes and challenges

  • Advocate for clients with agencies and service providers to minimize barriers to clients receiving needed services

  • Engage and collaborate with parents, guardians, siblings, and other family members to assess needs, provide supportive services, and strengthen family stability and client outcomes.

  • Actively participate in staff meetings, trainings, client reviews, internally and with partner agencies

  • Maintain confidentiality and comprehensive confidential case files

  • Establish and maintain collaborative working relationships with community resources 

  • Performs related duties as required. *

Qualifications:

Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:

  • Bachelor’s degree in a related field

  • 1 to 3 years of experience in related field

  • 2 years of experience working with high risk or probation youth

  • Commitment to the mission and values of the non-profit sector, with a passion for social impact and community service

  • Must have reliable transportation, a valid CA Driver’s License, and proof of auto insurance 

  • Must be able to successfully pass Live Scan (background check)

  • Required TB Test clearance

  • Good written and verbal communication skills

  • Maintain high level of confidentiality

  • Handle public contact and relations effectively

  • Develop and maintain effective working relationships
